Your First Visit: Day 1 of the Transformation for You or your Child

We can hardly wait to meet you! We set aside an hour for your first visit so we can learn all we can about your concerns, and get to know you or your child. As part of this complimentary consultation we will also take diagnostic photos and x-rays, which the Doctor will discuss with you. Again, unlike some orthodontic offices, all of this is at no charge to you. Prior to this appointment, please take a few moments to complete the New Patient Form for you or your child, and the Smile Evaluation Form. Both forms can be found below.

Insurance Information

If your dental insurance has an orthodontic benefit available, we are happy to determine your eligibility and benefits for you. When making your appointment, please share your dental insurance information. We are happy to file all insurance claims for you, and correspond with your dental insurance as needed to make it a hassle-free process for you.
